If you have some hidden icons in main manu then if you change the wallpaper of the phone then these hidden icons becomes visible at same time they also visible in hidden icons.. it’s funny try this.

What I had was I accidentally touch a widget and moved it to another screen. At the time I thought I had the same problem as you, but totally different.
So 252 having this problem have you updated you Launcher and widget apps on play store. I’m. In the UK and the Play Store had updates for these apps. Widgets version 2.0.1 and Launcher 2.5.7 this may address your issue.
